  • A valid referral
  • CO-Pay
  • A fully completed patient information sheets
  • Any x-rays, CTs, MRIs (actual films and reports)
  • Exact dates of injury

At the time of follow-up visits please bring:
  • Insurance cards
  • CO-pays
  • Updated referrals
  • Any studies
  • An updated patient information sheet every 6 months

CO-Pays, referrals and current billing information are required prior to service or your appointment will be rescheduled to a time when they are available. It is your responsibility to bring these to the office.

All information on the patient information sheets, especially injury information, insurance information, and your identifying information must be accurate. Knowingly falsifying any of this information constitutes fraud and will result in dismissal from the practice.
